day 2. thanks to Jae im back to my little blog!
whats something good that happened? got me excited?
(very sad that it takes me quite a bit of time to dig through the negative narrative to find something positive to evaluate in the day) i was very excited about this new case transforming under my eyes. 4mo initially thought to have HLH turns out to have disseminated histoplasmosis. because i took the time to do a little bit of reading yesterday i was able to navigate some of the discussion around this case and keep afloat of what was going on. it is pretty good news for the family since a diagnosis of hlh at this age would imply some sort of stem cell transplant in the future. now the only hope is that treating histo will be successful and this pt will recover well. it is a good case that's giving me enough insight that i am hopefully able to remember the presentation of both of these diseases for future reference.
something that was hard?
always comes back to the same thing. having to compare myself with someone and feeling like i am falling short. i think i did a pretty good job of managing the team while abby was gone. but i am still unorganized enough to keep everything controlled. and i also am not at the spot where i can keep all the kids apart in my head. i don't have a clear idea where they all are in term of presentation. maybe creating an exercise of doing a one liner in my head to keep things straight.
i continue to be impressed with abby's style and competence. i agree with jae that she is the prototype for the 'smart' person, the type of person that was told all her life that she is smart. she is quick, able to pick up things on the fly, quickly parses things and analyzes situations. and she has a good memory for things. she makes for a really good clinician. in some ways it is hard to work with her because i keep comparing things, but on the other hand i can highlight my own strengths which may not always be in the same realm but are still valuable. i get to teach more!
something that i learned today.
goes back to the first questions i guess. really cool case with lots of good tidbits to learn. delved into some immunology and discovered just how much i should know in that realm. i need to find some sort of basic immunology text. (yay, impetus to study) all i can think about as i am trying to remember the day is just how tired i am, its all a blur. a big tired fat blur.
more tomorrow.
